We offer both In-person and Online Classes.
The class schedule below pertains to In-person classes only. Online classes will be completed at your own pace. Classes are covered by Medicare and most insurances. Co-pays vary by plan.
Pre-Registration is required.
Classes with less than 4 people registered will be cancelled.
Class Times:
Classes are held in the office for one whole day or two half days.
- 1 Day Class = 9:00am-4:00pm
- 2 Day Morning Class = 9:00am-1:00pm
- 2 Day Afternoon Class = 12:00pm-4:00pm

Medicare and most commercial insurance carriers require a referral for diabetes education. Either form needs to be completed by your primary care physician. Download the form you need and take it to your primary care doctor. Ask your doctor’s office to fax the completed form to (904) 272-9149. We will contact you and get you in the next available class that fits your schedule.
